Elementary VBS is for children going into 1st-5th grade and Preschool VBS is for 3-6 year olds entering preschool or Kindergarten. There is no Middle School Bible Study week 2. High Schoolers and Adults can sign up to volunteer here.

**Placement Requests: If you have multiple children in 1st - 5th grade they will automatically be placed together in a crew unless you request below that they be separated. Younger children in the Preschool VBS will not be allowed to join older siblings in the Elementary or Middle School VBS in order to provide the best and safest experience for all ages.

If you want your 1st - 5th grade child be with other family or friends of the same age range, or with a specific teen volunteer, please provide their names. Keep in mind that children are placed in crews of 5 children with one teen crew leader. These crews are part of a larger group of up to 25 children who rotate to each station together.

We will do our best to accommodate your requests, though they cannot be guaranteed. NO DAY-OF REQUESTS PLEASE!

By submitting this registration for your child(ren), you agree to the following:

Medical Liability:

I understand that in the event medical intervention is necessary, every attempt will be made to contact immediately the persons listed on this form. If I cannot be reached in an emergency, I give my permission to the physician or dentist selected by the activity leader to hospitalize, to secure medical treatment and/ order an injection, anesthesia, or surgery for my child as deemed necessary. I understand all reasonable safety precautions will be taken at all times by St. Martha Volunteers & Staff and its agents during the events and activities. I understand the possibility of unforeseen hazards and know there is the inherent possibility or risk. I agree not to hold St. Martha Catholic Church, its leaders, employees and volunteers liable for damages, losses, diseases, or injuries incurred by the subject(s) of this form.

Conduct Agreement:

I understand that by signing this form I/my child agree(s) to cooperate and participate fully, that I/my child will show respect for the property visited, respect for neighbor, that I/my child will show respect for the law and practice safety skills at all times. By failing to meet this code of conduct, I/my child am/are aware that appropriate action may be taken and arrangements may be made for immediate removal from the event.

Photo Liability:

Unless I have denied permission for photographs on my child's KidCheck profile, I hereby authorize the making of photographs, motion pictures, videotapes, voice recording, internet distribution or other memorializing of said event and my child’s participation therein, and the publication and duplication or other use thereof. I hereby waive any rights to compensation or any right that I otherwise might have to limit if to control such making or use.
